No. This site lists the plans we are a provider of nutrition services for. We do not have information about benefits or payment levels.
How do I get help to find out my health care coverage and explanation of benefits?
To get the information you need about your coverage, contact the customer service department at your health plan through the phone number on your member ID card or in your enrollment materials. Here are key questions to ask:
- Do they accept the codes we use for medical nutrition therapy, which are 97802, 97803, and 97804?
- If not, what codes are acceptable for nutrition counseling?
- Does the plan cover the specific reason or diagnosis for which you plan to be seen by our office? For example, if you are overweight, will it be covered? Is it only for medical conditions?
- If they cover the above, how many visits per year are allowed?

24-Hour Cancellation Policy
Keatley MNT enforces a 24-hour cancellation policy for all appointments. To reschedule your appointment, notify the Keatley MNT Team at 1-800-571-8276 as soon as possible to avoid being charged in full for the missed appointment. Canceling appointments with less than 24 hours’ notice will result in a full charge. The client will be individually charged, not their insurance company. Please note that all Monday appointments require Friday notification. We value your patronage and will always do our best to accommodate your needs.
